The Way Home is a children’s complex care agency supporting children and young people aged 0–18 with complex physical healthcare needs across Liverpool and Merseyside.
Why our model is different
We prepare the team before care begins.
Previous experience is valued, but it does not automatically mean someone is ready to care for a particular child. Each team member completes structured learning, supervised practice and competency assessment before working independently.
Parents know their child best and are respected partners. They should never carry responsibility for professionally training or signing off the staff entering their home.
Hospital-to-home care in Liverpool
Where commissioned and agreed, a child’s proposed Liverpool care team may begin preparing while the child remains in hospital. This creates child-specific understanding and continuity before the move home.
The objective is not discharge at any cost. It is a safe and sustainable way home.
Support may include
Complex clinical care
Enteral nutrition, medication, seizure support, enhanced observation and specialist equipment.
Day and night care
Waking-night or sleeping-night support and assessed staffing ratios where appropriate.
Everyday participation
Support at home, in education, at appointments and in the community when commissioned.
Clinical oversight
Named clinical leadership, current care plans and ongoing competency review.
Individual assessment
Every child’s needs, risks, environment and team are considered before care is accepted.
